CANDIDATE FORM INSTRUCTIONS

New Registration: This relates to the use of the form for on-campus recruiting. If this is a new registration form check mark this box, otherwise, check mark the box labeled "Update".

Relocation Availability: This section tells employers whether you are willing to relocate. Write "open" if you can relocate anywhere the job requires. Otherwise, be specific. Examples: a) Michigan Only; b) Midwest.

Employment Objectives: Clearly and concisely state the type of position you are seeking. For example: to obtain a position teaching grades K – 6 or seeking a position teaching secondary mathematics and/or physics.

Cumulative Point Average: This is your cumulative GPA at EMU for all work completed.

Academic Highlights, etc.: In this section include any academic honors you have earned, scholarships, positions and memberships in student or professional organizations and any other activities (since high school) in which you played a leadership role.

Field Experiences: In this section, and in the following order, provide information about your pre-student teaching, cooperative education placements (if any) and volunteer efforts. Each of these should be listed on a different line and should include the type of experience, organization, city and state, grade level and subject or age group, semester and year in that exact order. Example: Pre-student teaching: Ann Arbor Public Schools; Ann Arbor, Michigan; Grade 7 – English; Fall 1990. Student Teaching: Ypsilanti Public Schools; Ypsilanti, Michigan; Grades 5 – 6; Winter 1991. Co-op: Mother Goose Preschool; Gary, Indiana; 3 and 4 year olds; Summer 1989. Volunteer: YMCA Recreation Youth Program; Muncie, Indiana; 8 – 12 year olds; Summer 1990.

Name/Address of Relevant Employment: List your most recent employment first and your other two most recent jobs in reverse chronological order. Provide the name of each organization for which you worked and the city and the state in which each is located. You need not provide the street address. Using "action" words (coordinated, supervised, operated, etc.) briefly describe your work. Be careful to use accurate verb tenses.

Special Skills: Indicate specific skills you offer that are related to your employment objective. Examples include: Foreign languages, computer programming languages, coaching ( mention special sports), etc.

Professional Certifications: Enter the title and date of a teaching certificate. Be sure to include your endorsements. Example: Michigan State Elementary Provisional Certificate K-8 with endorsements in Special Education K-12, December 1990; Michigan State Elementary Provisional Certificate K-6, April 1991; or Michigan State Secondary Provisional Certificate 7-12, December 1990.

Autobiographical Statement: This is one of the most important sections of this form. Employers analyze this statement to determine what type of employee you would be, how you would fit in with their organization, and if you have good communication skills. Watch out for grammatical errors, sentence structure, and punctuation.
